Vermedx Gets New Venture Capital Investment

Burlington, Vermont  --  Healthcare company Vermedx has raised $500,000 in series A round of venture capital funding.

Vermedx has created programs that it says "embody the Chronic Care Model (CCM)." This model has been developed to improve health outcomes by emphasizing proactive approaches by healthcare provider teams.

Fresh Tracks Capital led the round, which included other undisclosed investors. The company said it will use the funds to "enhance marketing efforts and continue to develop products."
