Care Team Connect Integrates With Michigan Network

   Evanston, Illinois  --  Healthcare software company Care Team Connect completed its system integration with the Michigan Health Information Network.

The network is the Michigan state initiative to improve healthcare efficiency and safety through electronic exchange of health information.

Upon initial launch, the Care Team Connect system will provide real-time updates and alerts for up to 25,000 of the 120,000 Michigan Primary Care Transformation Project patients.

Care Team Connect's platform ensures care coordination protocols are followed and provides real-time alerts to care coordinators.
